Primary Care Track Membership
Now taking patients ages 12-64!
Primary Care Membership allows for unlimited access to your physician. The Patient Hub/Portal is used for most all communication. Patients are seen in the office initially for a one-hour visit in which a detailed history is taken, labs likely ordered, and preventive screenings are recommended. Then, follow ups can be a phone call, message through the portal, telehealth, or in person. Any acute issues that arise will be addressed with same or next business day appointment, and these appointments generally run up to 30 minutes long.
This track is great for the busy person who is open to different forms of communication and does not want to stop their own day to come down to the office. Monthly payment will be collected, and there is never a copay.
