Log onto the Atlas MD Patient Hub/Portal for access to your physician, notes, labs, and more. Our practice uses Atlas MD, a HIPAA compliant Electronic Medical Record. We encourage you to always use the Portal for any concerns or issue — it’s the fastest, safest way that maintains a record for all.

NEW PATIENTS
All patients, whether those seeking Membership or Pay-Per-Visit, will need to contact us first via email at Hello@laddertothestars.blog or at 850-359-1519.
From there, Dr. Brooks will be in contact to discuss your needs.
When a plan is in place, you can Enroll online via our Enrollment Form and Patient Agreement. Please be specific! It’s a long form but we need to know your story. Our practice uses Atlas MD EMR, and the enrollment form loads straight into the EMR.
For those interested, copies of our Patient Agreements (which all patients are required to sign electronically prior to any services rendered) can be found below. There is no need to print them, as they will be filled out electronically.
